— Andrew Guthrie, Ph.D., R.P. is responsible for the personal health information collected in his practice

— Personal health information is handled in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and the standards of the College of Registered Psychotherapists of Ontario

What is collected

— Through this website: only what you choose to send — your name, contact details and a brief message

— In therapy: information needed to provide care, such as your history, session notes and billing records

How it is used

— To respond to your inquiry and arrange appointments

— To provide, document and evaluate your care

— To issue receipts for insurance and billing

— Your information is never sold or shared for marketing

How it is protected

— Clinical records are stored securely and kept confidential

— Records are retained for the period required by professional standards, then securely destroyed

— Email and website forms are not fully secure — please share sensitive details by phone or in person

Limits of confidentiality

— Where there is a serious risk of harm to yourself or others

— Where a child may be in need of protection, as required by law

— Where a court order or subpoena requires disclosure

— To report sexual abuse of a client by a regulated health professional, as required by law

— Otherwise, information is shared only with your written consent

— The site may use basic analytics to understand how pages are visited

— No personal health information is collected through analytics
